This course defines advance care planning and discusses why it is important, explores the legal standards for advance care planning as well as the essential elements of advance care planning, including reflection, conversation and documentation, and how you can support patients and their families through these phases. In addition, the course covers DNR (do not resuscitate) orders and POLST (Physician Orders for Life-Sustaining Treatment).

Objectives:
1) Explore legal and practical considerations for advance care planning
2) Discuss the healthcare provider’s role in advance care planning

This course was designed for all applicable settings in accordance with the organizational training standards for advance care planning outlined in the federal Patient Self-Determination Act (P.L. 101–508).

This course has also been certifed by ACHC, and verified to align with the CHAP: Hospice Standards of Excellence HPFC.2.D 10 and HPFC.10.I in Hospice Patient/Family-Centered Care; and Home Health Standards of Excellence APC.7.I.M2 18 in Assessment, Planning and Coordination Standards.
