This course gives students an overview of medical co-morbidities that can be found in the post-acute care setting, focusing on hematologic problems. With patient acuity changing in this setting, patients with the potential for multiple medical co-morbidities are more likely to be admitted. Examples of medical co-morbidities associated with anemia and anticoagulation will be shared, along with clinical pearls on focusing assessment and interventions on managing the complex patient. This course is appropriate for licensed nurses working in any setting, but particularly targets those in rehabilitation, long term care and home health.

Chapters & learning objectives
1. Anemia
This chapter reviews a variety of hematologic conditions that are often seen in the patient population in the post-acute setting, beginning with anemia. Hematologic conditions that are seen in post-acute care often lead to patient complications including bleeding or abnormal clotting. Nurses have the responsibility of possessing the knowledge and skills to care for patients with hematologic conditions. Key features of assessment and intervention will be included in this and the following chapter.
2. Anticoagulation
This chapter addresses complications that can arise from reduced clotting function. Key considerations for management of this condition in the post-acute care setting will be addressed.
