Technology, Software, and Electronic Medical Records (EMR) or Electronic Health Records (EHR) have become a part of our day-to-day operations. For the Home Care clinician, this provides both a wealth of information at their fingertips, and a new tool to utilize while assessing and caring for patients. However, for many this is a significant transition from the paper world they have been used to. For others, it is a transition from their current software to something new. For Administration/Leadership and Managers, it requires considerable research, preparation and support. This course is designed to provide tangible tools for both Leadership/Managers as well as clinicians that can be used to prepare for the most successful implementation or transition.

Chapters & learning objectives
1. The Mindset
Clinicians that have been using computers since childhood have a different mindset than those that have never used them or have limited exposure. This chapter will explore those differences and how those differences can impact the success of your implementation/transition. We will address tips and tools that can be used to guide you in avoiding pitfalls.
2. Knowledge is Power, Embrace The Keyboard
This chapter is specifically designed for those clinicians that are not as tech savvy. Learning how the computer works, what a window is and how to access the internet can be overwhelming. When combined with trying to learn an EHR system, it can stop a clinician in their tracks. This chapter will address some of the basic computer and software language and resources.
3. Setting the Stage for Success: Leadership/Manager Keys
Planning to fail is accomplished by failing to plan. Therefore, as leaders/managers, it is critical that thorough research be made into the needs and wants of your agency before embarking on a software implementation. After such research, a strategic plan must be put into place that best fits your agency and clinicians to ensure everyone’s success. This chapter will highlight many aspects that need to be researched about your new software. We will also discuss several strategic plans you can utilize during the implementation to best ensure success for your clinicians and thus your agency.
4. Ensuring your Individualized Patient Care Plan is Created and Maintained to the Highest Standard
With the latest CoP updates, having a patient-specific care plan has never been more critical. Your software must accommodate the creation and maintenance of this care plan in a manner that is conducive to the operations of both your clinicians and agency. This chapter will focus on care planning examples, with critical thinking, that show a process and the efforts toward the individualization of the patient’s care.
5. Team Work
Clinicians are used to working together as a team. and this is critical in the success of your implementation/transition. This chapter will address tips that each clinician can use to help support the rest of their team while everyone is learning the new system.
6. Celebrate Your Milestones
Everyone loves to celebrate and be celebrated. Learning a new EHR is a challenge for even a seasoned software user. Leaders need to include celebration as part of the strategic plan, and clinicians need to be prepared to celebrate each other. This chapter will address some of the expected milestones and ways you can help each other celebrate your transition.
