Older adults and individuals with neurologic conditions are at an increased fall risk. The ability to produce quick reactive stepping is a critical component to decrease this risk and improve function. Perturbation-based balance training (PBBT) is an innovative intervention design with a goal of improving control of the needed rapid balance reaction. An abundance of PBBT research has been performed with a variety of patient populations, including stroke and spinal cord injury, as well as aging adults. However, the research design of these studies is often impractical for the everyday clinician and doesn’t address the preliminary building blocks for varying levels of patients in different settings. In this course, the speaker will review the necessary components to progress different patient diagnoses/settings to effectively perform PBBT. Through lecture and video-based cases, this session will focus on translating the evidence to successfully implement a progression of PBBT-type interventions for the above patient diagnoses. The presenter will emphasize key evidence-based concepts along with practical solutions to execute PBBT interventions. The purpose of this course is for participants to leave with practical and evidence-based insights to safely and effectively apply PBBT with patients when returning to their clinic.

Chapters & learning objectives

Literature Review for Falls and Perturbation-Based Balance Training (PBBT)

1. Literature Review for Falls and Perturbation-Based Balance Training (PBBT)

This chapter will provide a comprehensive review of the priority and latest literature regarding the importance of PBBT. This will include a variety of diagnoses and the impact of the different types of implementation of PBBT.

Outcome Measures to Utilize With Perturbation-Based Balance Training

2. Outcome Measures to Utilize With Perturbation-Based Balance Training

This chapter will provide a comprehensive review of all of the relevant standardized outcome measures that can be used to assess a person’s appropriateness for PBBT. The performance of each measure will be discussed and/or demonstrated to allow ease of replication. The appropriate level of patient will also be incorporated. In addition, documentation strategies and suggestions will be reviewed for optimal reimbursement.

Review of a One-Session PBBT Option

3. Review of a One-Session PBBT Option

This chapter will review the literature on the potential benefits of performing a one-session PBBT intervention. This will include the varying ways to provide this type of session and the possible results.

Aging Adult Case Examples

4. Aging Adult Case Examples

Video cases for a variety of aging adults will be presented. Utilization of the Rate of Perceived Stability Scale will be incorporated into a circuit training design, using activities that emphasize anticipatory and reactive interventions.

iSCI Case Examples

5. iSCI Case Examples

Video cases for a variety of people with incomplete spinal cord injuries (iSCIs) will be presented. Emphasizing the results from the outcome measures performed in this chapter will align the interventions with the priorities seen in the outcomes while emphasizing the patient’s functional interest.

CVA Case Examples

6. CVA Case Examples

Video cases for a variety of people with CVA will be presented. An example of an intervention design will utilize the Activities-Specific Balance Confidence (ABC) Scale. In addition, options to increase safe challenges in the home with affordable and portable equipment will be provided.

Question and Answer Session

7. Question and Answer Session

This chapter is a question and answer session. The question and answer session is facilitated by J.J. Mowder-Tinney.
