Hospice HOPE Tool Explained

A big change is coming for hospice providers. On October 1, 2025, the new patient assessment tool from CMS, called “HOPE,” will go into effect. This new tool utilizes new assessment-based quality data to enhance the Hospice Quality Reporting Program (HQRP). Now is the time for hospice clinicians to learn how to use the HOPE tool and prepare for how it will change their practice.

On October 31st, 2024, Jennifer Kennedy, EdD, MA, BSN, RN CHC, will discuss the new regulatory changes coming with the HOPE assessment tool and how it will contribute to the patient’s plan of care. She’ll be joined by Anna Zimmerman, MSW, Sr. Program Manager, Home Health & Hospice, to explore opportunities for hospice clinicians to adjust their practice and prepare for the new regulations.
