We’re halfway through the second performance year of the Home Health Value Based Purchasing (HHVBP) program, and there’s a lot to keep track of—three new metrics are being added, while four metrics will be removed beginning in 2025. If that wasn’t enough, agencies will receive this year’s results
that will impact payment adjustments in 2026, and prepare for payment adjustments in 2025 based on their performance last year! While it might sound daunting, we have strategies that will help you prepare your program for the rest of this performance year and beyond.

On Tuesday, July 16th at 1PM EST/10 AM PST, Charles M. Breznicky Jr., RN, MSN, MBA, HCS-D, Clinical Director at SimiTree Healthcare Consulting, will be joined by MedBridge Head of Clinical Strategy Joseph Brence, PT, DPT, MBA, FAAOMPT, to review the current and upcoming HHVBP Claims-based and OASIS-based metrics, including Potentially Preventable Hospitalizations, Discharge to Community, and Discharge Function Score. They’ll discuss the similarities and differences between the current and upcoming metrics, along with steps agencies can take now to improve the delivery of care in these areas and increase success in HHVBP.
