Social determinants of health directly correlate to health outcomes and contribute to persistent health disparities. Screening for social determinants of health allows clinicians to identify the social determinants most impacting health of their patients, allowing clinicians to link patients to needed resources, and informing health system policies and interventions. However, screening for social determinants of health is new and uncomfortable for many clinicians.
