Social determinants of health significantly impact health outcomes and disparities but can be challenging to conceptualize and address in clinical settings. Strategies to identify and address social determinants have the potential shape our approach to healthcare in a way that centers patients’ lived experiences and improves health outcomes. Participants will gain a comprehensive understanding of social determinants of health along with tools and frameworks for identification of social determinants, intervention, and outcome management.
