Heart failure patients have difficulty managing their disease due to personal barriers, such as lack of knowledge and inadequate support systems. This includes both social support and healthcare inadequacies, such as lack of access to providers and comprehensible materials. The inability to successfully self-manage the disease leads to rehospitalizations, financial burdens, caregiver burnout, and poor quality of life. This course will describe common barriers experienced by heart failure patients and provide cllinicians with assessment techniques and interventions to address patient needs and promote independent self-care.
