HHVBP: Success Strategies for 2025 and Beyond

This course series will provide an overview of the HHVBP changes coming in 2025. Clinical courses will review, in more detail, the impact of these changes on clinicians and actions they can take to improve the claims-based, OASIS-based, and survey-based measures. The metrics involved in the model and their respective weights will be addressed, as well as the calculation for an agency’s Total Performance Score. Leadership courses will review how to track and analyze data. In this course, learners will be provided with steps they can take to succeed under HHVBP. Managers and frontline staff will come away from these courses with a deeper understanding of HHVBP and their impact under this new model.
