This course will focus on specific interventions required to improve outcomes for skilled nursing facility (SNF) or nursing facility (NF) residents with cognitive impairment or dementia. Nearly half of nursing home residents have Alzheimer’s disease and related dementias (ADRDs). Those residents are hospitalized two to three more times as often as residents without dementia This course will review the unique needs of residents with dementia during transitions, with a focus on prevention of unnecessary hospitalizations. Nursing staff will learn the essentials of comprehensive assessment of residents with dementia, identifying risk factors for hospital admission, and managing clinical data during transitions of care. This course will help the SNF and/or NF to advance their reputation in the community by providing quality, cost-effective care to residents with cognitive impairment or dementia.

Chapters & learning objectives
1. Impact of Dementia on Transitions of Care in the Nursing Facility
This chapter will review the common causes of dementia and their typical presentations in the NF. Nursing staff will learn about the characteristics of residents with dementia that place them at high risk for hospitalization. The focus will be on reducing barriers to quality transitions of care for residents with dementia, which include poor communication or incomplete transfer documents.
2. Comprehensive Assessment and Management of Residents with Dementia or Cognitive Impairment
This chapter will discuss essential quality care for residents with dementia, with a focus on assessment, behavior management, medication safety, symptom management, and regular assessment of decision-making capacity. Specific roles of interdisciplinary team members will be examined to keep the resident at the highest level of function.
3. Conversation With SNF Staff About Transitions of Care and Residents With Dementia
