This course will explore common disease states that healthcare providers may encounter that lead to patients requiring prolonged mechanical ventilation. The course will discuss how these disease processes may involve a variety of approaches to successful mechanical ventilation management. This course will also discuss planning an approach to ventilator weaning and liberation of the prolonged mechanical ventilation patient when appropriate. The goal of this course is to prepare guidance for multidisciplinary, team-based care of patients with long-term mechanical ventilation.

1. Population of Prolonged Mechanical Ventilation
This chapter discusses the growing population of patients requiring prolonged mechanical ventilation and the multiple disease states that encompass this population. Patients with prolonged mechanical ventilation can be cared for at different facilities with variable resources and expertise. The approach to patients with prolonged mechanical ventilation should best be individualized in context of medical, environmental, and social factors.
2. Technical Aspects of Ventilation
Ventilators can be equipped with a number of different circuit and exhalation configurations. Secretion clearance therapies may be adapted to the ventilator circuit. It is important for the family and the caregivers to be aware of the common problems in home ventilator management. Working with durable or home medical equipment (DME/HME) companies is important in the care of these patients.
